Course forms

Academic Programs oversees changes in currently offered programs and the creation of new programs, both of which require the approval of the Provost's designee, Associate Provost and Associate Vice President, R. Darin Ellis. Once approved at the Provost level, the program changes or new programs are submitted to the Board of Governors for final approval. All programs are taken to the Michigan Association of State Universities for review by academic officers representing all 15 public universities in Michigan.

David McGrann in Catalogs and Course Records accepts New Course, Change in Existing Course and Course Materials Fees forms and channels the forms to the correct destination (Graduate School, Undergraduate Programs, Registrar, etc.) and maintains records of all course offerings.

Directions for completion and submission are included on each form.
